On January 8, 2009, approximately 2055 mountain standard time, a Robinson Helicopter Company R-44 II, N168SH, impacted terrain following a loss of control in cruise flight near West Valley City, Utah. The flight instructor and his pilot certificated student/owner were seriously injured. The airplane sustained substantial damage. Majestic Helicopters LC was operating the helicopter under the provisions of 14 Code of Federal Regulations Part 91. Night visual meteorological conditions prevailed for the departure of the instructional cross-country flight from Tooele, Utah, which had originated approximately 10 minutes before the accident. Instrument meteorological conditions prevailed in the vicinity of the accident site at the time of the accident. The two pilots had not filed a flight plan, but they said their destination was Bountiful, Utah. The two pilots said they were on an Instrument Flight Rules (IFR) training flight when they inadvertently encountered obscuring weather. The student said that he was flying the helicopter and entered the weather. He said the helicopter began spinning and the instructor took the flight controls. Neither pilot remembered impacting the ground.
Aircraft yawed on take off, worong pedal applied, yas rate increased. Aircraft rolled over during attemped landing.

3 qualified pilots on board, at the end of a few days of mountain flying instruction. Due to weight hover only possible IGE, take off into wind not possible so took off downwind. The aircraft hit tubulence and then slid along ground and rolled over. Aircraft was over HOGE weight and under HIGE weight for the conditions of the day, the attempted transition in ground effect was unsuccesful.

At a height of approximately 15 feet above ground level a large industrial bag was blown up into the main rotor blades. The pilot executed a forced landing in a nearby open area, after landing on inspection of the helicopter it was noticed that the main rotor blades were damaged. Nobody was injured in the accident.
