Narrative ENG FAIL. DISTRESS CALL. STRUCK LEVEL GRND. ALL 6 ON BOARD KILLED.
DetailsAFTER PASSING OVERHEAD THE OXFORD NDB THE PILOT TRANSMITTED A DISTRESS CALL TO THE EFFECT THAT THE A/C HAD SUFFERED ENGINE FAILURE.SHORTLY AFTERWARDS ALL CONTACT WAS LOST.THE AIRCRAFT WAS SEEN TO EXIT FROM LOW CLOUD FAIRLY RAPIDLY, STRIKE THE GROUND AND IMMEDIATELY BURST INTO FLAMES.ALL SIX OCCUPANTS WERE KILLED ON IMPACT. SEE ALSO 86/01159 & 86/01532 WHICH REFER TO ENG FAILURES ON TWO OTHER AS355'S UNDER SIMILAR CIRCUMSTANCES. CAA CLOSURE:ICING/SLUSH IN ENGINE AIR INTAKES IS ASSUMED TO HAVE CAUSED DOUBLE ENGINE FLAME-OUT WHICH RESULTED IN AN IRRECOVERABLE LOSS OF ROTOR RPM. CAA EMERGENCY AD14-10-86 REQUIRES COMPLIANCE WITH DGAC AD86-153-31 (B) WHICH PROHIBITS OPERATION OF THE A/C IN CONDITIONS OF VISIBLE MOISTURE AT AN OAT BELOW +5 DEG C,UNLESS AN ACCEPTABLE ENG AUTO RE-IGNITION SYSTEM IS FITTED.APPROPRIATE AMENDMENTS HAVE BEEN MADE TO THE OPERATIONS MANUAL. CAA RESPONSE TO EIGHT AAIB SAFETY RECOMMENDATIONS IS CONTAINED IN FACTAR F7/88
